Understanding Frailty and Assessment Tools
- Introduction to frailty and why it matters in domiciliary care
- Use of the Clinical Frailty Scale (Rockwood)
- Practical demonstration of frailty assessment using case studies
Interactive Exercise
Trainers will practice assessing a frailty case and discuss findings using the Clinical Frailty Scale.
Identifying Illness, MDT Access, and Nutrition
- Recognising early signs of illness in frail clients
- Using the PINCH ME mnemonic for identifying deterioration
- Collaborating with multidisciplinary teams (MDTs) for effective care escalation
- Understanding nutrition's role in preventing frailty progression
Interactive Exercise
Scenario-based discussion on recognising illness using practical tools and how to communicate with an MDT.
Medication and Polypharmacy in Frailty
- Understanding polypharmacy and medication risks in frailty
- Identifying medication-related deterioration and when to escalate
Interactive Exercise
Trainers will work in groups to review case-based medication plans and identify risks and escalation points.
Advance Care Planning and End-of-Life
- Advance Care Planning (ACP) for frail clients
- Using ReSPECT Forms
- Recognising last days, weeks and months and ensuring dignified care
- Supporting domiciliary carers' mental health after a client's passing
Interactive Exercise
Trainers will discuss strategies for discussing ACP and end-of-life decisions with clients, families, and healthcare teams.
